The national Keep Well Project is an example of anticipatory care in practice, developed as part of plans to tackle health inequalities in Scotland. Keep Well aims to increase the rate of health improvement in 45-64 year olds in areas of greatest need.
Salus Case Management services provide a specifically developed service for clients of Lanarkshire’s Keep Well Project.
A Case Manager will complete a comprehensive assessment; develop an action plan and co-ordinate treatment. The assessment includes a range of pre programme measures that are repeated mid programme and/or on completion to evidence the client's progress. The programme provides quick access to the following services where appropriate:
- Counselling
- Physiotherapy
- Bowen Therapy
- Occupational Therapy
- Psychology
They may also provide signposting to services for help and advice on issues such as employment, housing and debt. All treatments and interventions are arranged in local community venues whenever possible.
To be eligible clients must be aged 45-65 years old and be registered with a GP who is participating in Lanarkshire’s Keep Well Project.
Clients may be referred to Keep Well by:
- Keep Well screening nurses
- Counterweight staff
