MCN Stroke

The MCN Stroke programme is designed to assist clients below the age of 65 who are recovering from a stroke, to improve their health and move closer to, or return to the labour market.

The programme is working in partnership with the Managed Clinical Network for Stroke in Lanarkshire, to offer individual per-employment packages to those who are motivated to get back to employment, training or further education.

A Case Manager will complete a comprehensive assessment; develop an action plan and co-ordinate treatment.  The assessment includes a range of pre programme measures that are repeated mid programme and/or on completion to evidence the client's progress.  The programme provides quick access to the following services where appropriate:

• Counselling
• Physiotherapy
• Bowen Therapy
• Occupational Therapy
• Psychology

All treatments and interventions are arranged in local community venues whenever possible.

The Case Manager may also provide signposting to services for help and advice on issues such as employment, housing and debt.  The Case Manager will look to source and provide information, help, support and signposting to services for any aspect of the client's health or social circumstances which may be acting as a barrier to a return to work, training or further education.

To be eligible clients must be:

• Motivated to engage in employment
• Of working age

Clients can be referred to the MCN Stroke Programme by:

• Stroke Nurse
• Young stroke support worker

The MCN Stroke programme provides clients with access to several free, confidential services and aims to refer to services quickly thus helping to prevent further deterioration of health problems, whilst supporting the client throughout.  The Case Manager liaises with a variety of services and relevant professionals to ensure effective outcomes.
